Community Based Transition Center Referral Process


CBTC Mission Statement
:

Bridge the gap between High School and Adult Life by providing opportunities for students to gain skills and community connections needed for a successful transition.


The Community Based Transition Center provides transition services to IEP students eighteen through twenty-one who require specially designed introduction to meet their transition outcomes as identified on their IEPs.

Using the community as the classroom, possible skills to be developed and applied include:   Community Access, Self-Advocacy Skills, Social Skills, Functional Academics, Vocational Training, Work Experience, Job Searches, and Independent Living Skills.

CBTC Referral Process

1.   The IEP team defines unmet transition needs that may extend beyond high school.   Goals and objectives are developed to help the student gain those skills.  A referral to the CBTC is appropriate for adult students 18 or older who cannot meet their transition outcomes at their high school.
  *It is expected that extensive transition services be provided as part of the high school program starting no later than age 16.  Many special education students should achieve the training and connections needed prior to the end of high school and graduate on time.

2.   If the adult student is interested in applying for placement in CBTC, the referral packet is distributed.  The sending school, student and parent have a section of the CBTC referral application to complete.  The completed referral packet will be returned to the school psychologist, who will share it with the staff at CBTC.  Then the parent and the student sections are part of this webpage.  Staff can access the full referral packet on the Special Education Astro network. 

3.   If after reviewing the completed referral packet with CBTC staff, the IEP team meets to consider the referral.
